Microsoft Dynamics AX (Archived)

Cannot create a record in Action state (HcmActionState). Personal Action number: (system generated number)

We are getting following error when creating any new employee or try to modify any employee in Human resources module Dynamics AX 2012 R3

But when we try to find the same Action Number in Action list we are unable to find that.

How can we deal with this error, as for as i think it should be auto incremented, but in our case some times it is not happening and we are getting this error time to time.
