I have the admin rights.
Any idea about this?

Security role is enabled.
It is visible to everyone.
I have the admin rights of the system. The form was a replica of another form. So, no issues at the moment.
The only concern is, it was fine before upgrading to V9 and now it shows NOT FOUND.

Seems you have multiple form in your account entity but the default form you don't have assigned security role.
I would suggest go to customization > entity> form > in the button area you can find the button enable security role - > you need to cheked assigned user security role.

I have not deleted the form by any chance. Its opens fine in incognito mode because I changed the form order. The form named "Information" shows http 404 Not Found.
Can you check if you get the error in incognito/ inprivate mode? Sometimes when you delete the form, the references to that form cached at the browser level and cause this issue.

I checked. Now the form with the id in the log file shows http 404.
I have changed the form order and now its opening. But don't know how the form got disappeared.
I had 3 forms in Account. Information, Informationold and Account.
When I open Information, it shows http 404. Other two forms open fine.

Yes the log file says systemform with ID="abc" does not exist.
But when I opened the form with the id, it is the Account default form in the production environment.

When you download the log file, does it say which record etc? There may be some business rule, workflow which would be using a record which is not present in your sandbox.
II remember I got the similar one in my case I was setting the default currency/ price list.
