One of my development team tried to fix a bug they were having by adding some dlls to the CRMWeb folder.
This caused all of the Orgs on that UAT server to present with a 404 error. He backtracked the changes and the error persists.
We have tried IISresets, server restarts, Used files from the backup/snapshot from few days previous, deployment manager disable/enable for orgs, service restarts.
The server is IFD and on trying to access I get the login prompt. Upon completion I get the 404 error.
Have tried Fiddler and checked Event Viewer but can't find any more info to go off.
I have also side by side cross checked file size and version numbers of the dlls in that folder against the previously working snapshot.
The next thing I was going to try was to go through the IFD setup again see if a connection/setting has been messed up and needs resetting.
Any other thoughts appreciated

Just adding can get to both the Discovery and Organization service ok
Repair didn't work.
We are going to uninstall / reinstall tomorrow and hope for the best.
Any other theories greatly received.
As an aside the Outlook plugin errors with the following trying to connect, probably a red herring?

Why would it be trying to connect to http, when I try to browse to that url it errors, if I do with https it is fine. Does that suggest that somewhere a link is incorrect?

Messing with assembly (dll) files on the CRM Server can break the product.
It is very possibly that this has to do with versioning of files for example or updates that are not supported by the product.
It is too hard to pinpoint the actual issue without having any data to analyze.
As Wayne suggests, a reinstallation (or possibly just a repair) of the CRM Server would probably be your best bet, as that would override the current data bits (assemblies) on the CRM server.

Do you know what is likely to have been broken by the CRMWEB folder having files added to it / amended.
The quickest way to fix that would be to document your IFD settings and reinstall your CRM server.
