Planning is ignoring item coverage settings

D365 F&O 10.0.28 (10.0.1265.78)

Setup safety stock via item coverage min/max. Not creating planned orders and ignoring lead time. I have configured this in numerous other client instances. Planned orders are created from other sources, project item requirement, manual forecast demand, but not safety stock

- Zero inventory for item

- Setup up a forecast demand for the same item and planned order is created

- Set item requirement on a project and planned order is created

- Where planned orders are created, lead time is being taken from default order setting rather than the override in item coverage

- Tested with fulfill minimum set as today's date + min, todays date, and coverage time fence

- Coverage by warehouse enabled

I am thoroughly baffled
