Missing Purchase Order confirmation journal after Request change

Dear Experts,

I am got stuck on problem, PO confirmation journal is missing on the 'Purchase order confirmations' screen and here is the scenario,

Using Active change management.

1. Created the PO and confirmed (Manual) after workflow approval - Journal is generated.

2. Request change -> Changed the PO language from Header -> Setup. PO getting automatically get confirmed after workflow approval (Not using auto confirm on workflow). Missing the version record in confirmation journal. Showing only the same generated by step 1.

3. Request change -> Changed one of the PO line column say 'Customer reference'. PO is approved through workflow and status is 'Approved' now. Able to generate the confirmation journal and showing the new version in confirmation journal screen as well. But it was version 3.

Can you go to Procurement and sourcing > Inquiries > Order posting > Confirmation
