Two Options - Flag Y/N selections and add to related Entity Form

Hi, here is the scenario I need help with:

I have a custom Entity 'Diet' in CRM 2013.  This will have a related entity to it, 'Hospitilization'.  Both of these are related to the 'Contact' in CRM.  Within the 'Hospitilization' entity, there will be checkboxes (because of the no multi-select functionality in 2013) for a list of 'Assistive Devices'.  If you are a user, you can choose multiple assistive devices there.  However, if the user selects an Assitive Device that is Diet-related, I want it to be appear as selected on the related Contact's 'Diet' form (as =Yes or =1, hence the checkbox will appear as selected on that form).  
Problem is, with a field format of Two Options, there is no way to flag the partcular Two Options field (for instance, Weighted Spoon, which is a Diet-related Assistive Device) as something like Is Diet = Yes.  If there was, I could use code/workflow to have the value moved over to the Diet form.  
Is there any way, either out of the box or with JScript or Plugins, that the above can be achieved?

    If my understanding is not wrong in short ,you have three entity Diet, Hospitalization, contacts. Each entity have contacts lookup in the forms.

    Now how do you know that particular device(checkbox field) related to Diet entity?

    For me in this scenario instead of checkbox you can implement subgrid of devices where user can add device data which are related to Diet.

    Now using you may have another subgrid in contacts forms where all the devices related to contacts, Hospitalization data will show using subgrid view filter criteria.

    Hi, thanks for the quick reply,

    Basically, if a user adds an Assistive Device on another entity, and it is a Diet-related Assistive Device, I want that item to flow through to the Diet entity as a selection made.  The problem I have is what you mentioned - how do I know that particular device is related to the Diet entity - i.e., how do I flag the Assistive Devices that are Diet related, since the field type is Two Options.  

    I considered adding an entity of Assistive Devices that are only Diet related - but there are already fields created and in use by users that are Assistive Device related.  So I don't want to create more confusion.  

    Does this make sense?

    Okay,  when user selects the predefined Assistive Devices checkboxes , you want the list of selected value to be  move into associted Diet entity record (which related to Hospitalization).

    For my opinion you can write a plugin in crate/update message of Hospitalization entity and passed those checkboxes which checked  (You  know which check box is which device) , prepare the list of devices and using for loop  update in diet entity( not sure you will need to update or create ) . Similar way update , create in contact entity as well.
