How to add custom form view to Appointment(EMR) entity

I have created the custom form in the appointment(EMR) but it's not showing the particular form which I have created as I can't able to view the drop-down in this entity.

Hello,

You would need to add Form in App Designer.

Navigate to Customization->Customize the system->Model Driven apps->Open your specific app->Add your custom form to this app and remove any which is not required.

Once you publish the changes and refresh. you will see your new form.

Hi,

Have your tried to clear your browser cache or tried to use another browser, the may be cache issue

Hello,

Make sure Form is active. and you have enabled required security roles for that form.
