File Attachment not showing after upload

I am trying to upload a document against a an Employee using the Discipline window.
After I click on the Attach button and browsed to pick the file, it doesn't show in the grid for me to preview and save

    Helly, this is Terry from Microsoft and I can help you.
     
    This window is a bit different from other windows, I want to make sure did you choose or save it with an Offense Number, such as mine below?
    Once I did that vs just attaching to the blank window, then it saves the note for me and I can view it, etc.
