Is there a report that shows the 1099 Box #, amount, and vendor name?

Is there a report or Smartlist that can be run that shows the 1099 Box #, amount, and vendor name? I know I can print the 1099's on blank paper to do this, but it's such a waste of paper and you still have to visually line up the box number.

    The easiest way to get this would be to create a custom SmartList using SmartList Builder or via SQL query.
