I want to create a simple form with Dynamics 365 trial version, but there's no form button.
I tried giving myself the system admin role, but it says permission denied (no matter what role I try to give myself)

Maybe this is a terminology thing. I think "forms" are just screens in Dynamics that you can tweak.
I want to create a form from scratch which is totally custom and ultimately creates a record. Is this considered an app? And therefore I need "App Designer". If that's the case I think it's a permissions thing

I don't have that. I have Settings -> Customizations -> Developer Resources.

I don't see anywhere to create a new solution. Maybe the trial version doesn't have it.

Where are you looking for Form button? You should navigate to settings --> customization --> Customize the System/Solution --> Expand Entity --> Forms.
