Entity Form Field for document upload

Hello,

I have created a new entity for medical records and have enable document management. How do I create a field on the form to enable users attach/upload a document to the medical record entity?

Hello Davesconxy;

There are two locations for document storage. If you have sharepoint you would use your sharepoint library to update this way. The second if you don't you would attach a file to a related note. That's how the documents are stored on an entity inside of crm.

Hello Jason,

I deleted the note from the form during customization. I no longer see the note field within the filed explorer. how do I add a note section to the form?
