Hi, in dynamic 365, is there a way to create checkbox on the form?

Just add two-values field (bit field), put it on form and change display style to checkbox.
Yes. You can create bool field (Two parameters), place it on a form, and on an element properties -> formatting you can chose need view (checkbox, picklist)
what about multiple selection
multiply selection is a pick-list anyway. It can be more than 2 entities in it.
Multiselect checkbox control is not available OOB at the moment.
If you want to have you have bunch of choices:

and many more. All you need is to type what you need in google search bar ;)
But I created two options field, it only displayed the checkbox without label?
It has to have label because it's a field. Otherwise you will have to develop Html/Js webresource that will show checkbox and put it on form.
No, I mean, it has the label for the field, but not the option label
I'm really sorry but what labels should be visible for "checkbox" field? I would agree that values should be visible for "Radiobutton" style but for "Checkbox" - it has no sense. Checkbox is checked or unchecked - this is how it works.
So, currently, it is showing like this? is it supposed to display two options?
