Labels Not Displaying on Certain Form Tabs

I have customised an entity form (Leads) using the Form Editor to include some new fields. Now there's a problem displaying the labels for 3 tabs within the form in IE - when I load the form there is no label for the following labels; General, Details and Notes & Activities. I have specified that these should show in the form editor and when I preview they are there.

Any thoughts as to why this is happening?

    I kept working on this one and tried a few combinations of the following theme. I changed the labels to contain different text to the default text, published the changes and now the labels are displaying on the form, strangely.
