Custom lookup in form control is not loading

Hello Everyone,
 
I've created a new Form (Lookup - Basic) and using it into another form (let's call it FormB) for lookup purposes. The lookup form involves a temporary datasource, and the records are populated during the form initialization.

When I open the lookup field from FormB for the first time, it works perfectly. However, after saving and attempting to use it for the next record, the lookup field control fails to load.
 
 
Refreshing the browser resolves the problem, allowing the lookup field to work for one more record(1st record after refresh).
 
I attempted to debug the issue, but the debugger isn't hitting on the lookup method itself. Any insights or suggestions on what might be missing or causing this behavior would be appreciated.

    Do you probably have a preview version of Dynamics 365 F&O installed on this environment? There was an issue, I thought with a preview of 10.0.38, where the lookups in general was having this behavior. If you have a preview installed, check if it will be fixed by installing the GA version.
