Hello Team,
I have created on form and hosted it on my word press site everything is working correct on form submit it is creating lead in my crm successfully but when i go to insight tab i found out that it is also showing created leads in Error also i m confused why this is happening.
can some one help me in this!

Please take a look at some of the reasons a submission could fail - docs.microsoft.com/.../failed-submissions
I suspect what you are seeing is multiple submissions with the same information and a failure due to that. The link above should have information on how to debug the issue and how to deal with such failures.
