We need to do three steps for a new user.
1. assign license;
2. assign role for security of D365 visiblity to show titles to user;
3. assign role in CRM to user.
Can 1 and 3 not be combined, or even 1,2,3 in Azure AD or On-Premise AD (less preferred but we use it as main on which Azure AD is synchronized instead of reversed (Azure AD leading).

Only with custom development. There's no native integration between Azure AD and Dynamics CRM. CRM Online uses Office 365's provisioning methods, not Azure AD. CRM On-prem only supports on-prem AD natively, to my knowledge.
There is no mapping in any of these of security roles to anything in AD, since they serve different purposes.

Not without custom tools and/or development... I don't think so.
I worked on something similar to this with an Azure AD sync to webroles and it required that
A. Web Roles and AD Security groups be the same name
B. Custom C# program to sync them.
