I read where adding <TEST> to the company name will pop up a message alerting one that they are in a test company.
How to add <TEST> to company name to Fabrikam if it has already been created? When I go to Setup>Company>Company the company name field is grayed out. Do I need to update a field in the database? If so, which one?

The graying out of Fabrikam is because this is the test company. I can't remember when it was changed so that we couldn't change the name of Fabrikam, but it is an attempt to keep folks from using Fabrikam as their production company. Restoring your database over Fabrikam doesn't change the name.
Back to the question, you would have to add <TEST> to Fabrikam in the SY01500 table in the DYNAMICS database, but why would you want to??? The system generates a warning message now telling you Fabrikam is the sample company. If you aren't getting any message, look at your dex.ini file. It could be omitted by a dex.ini setting.
If you do change the DYNAMICS database, you will get the Sample company warning first, and then the test company message.
