Hello,
A user reports that the header information on a lead form is missing. I am able to see the header info.
The form layout shows the header information is still there and everything is set to be visible. He has permission to view the form.
What else should I check?

I'll keep this list for reference.
The issue does seem to be browser related and random. The second user was able to close the browser and come back in to all headers visible. The first user I think just refreshed or closed then reopened the record and the headers returned.

If it is specific to a user, who is unable to see a header information, you can try following the mentioned steps.
1. Check in different browser, clear the cache and cookies of browser, try in InPrivate browsing and observe the behavior
As there was an issue with chrome browser with 72 version which were having such behavior
2. Ask user to switch the network and also check with the same user in different machine and then observe the behavior
3. If user is still unable to see header, post performing above steps. then you can check in debugger tool by using F12 key on keybaord and compare for working and non-working
This just happened to a different user on a case form.
