I found the issue. ToolBarEnu.dll was missing for some reason.
Have not run into this variation before but SL 7.0 and SL 2011 store menus as the individual user level in xml files and I suspect that the xml file has become corrupted. These xml files are stored on the local drive C under users\user name\appdata\roaming\Microsoft dynamics SL\menu cache. Get out of SL and delete the xml file with "menu' as part of its name and then go back into SL and see if that corrects the issue. SL will rebuild that xml file when you go back in.
