Same serial number shipped twice

Our client came across this issue. One serial number has been shipped twice. Is this possible?

When I checked the applied entries Entry 1655 is applied with 1646 and entry 1650 is applied with 1647. 

If anyone got an idea let me know. An urgent issue.

    This is not related to serial number creation. If you look into the screenshot. Even though the applied entries are different same serial number has been shipped twice.
