We are using MS-Dynamics CRM 4.0 with sql DB 2005 (Production server), we are in the process of upgrade version from 4.0 to 2015 version, so for the safer side we are planing to shift server with in same domain.
For your information: the production server has sql server 2005 version and new server has sql 2008 version.
Please provide me step by step procedure to fix this.

Doing a CRM 4 to CRM 2015 upgrade is difficult. I advise you to consider a data migration and a brand new development/customization in CRM 2015.
You'll need to pass through CRM 2011 and CRM 2013 SP1 in order to complete the process.
