Recently, around 5/6 (at least that is when I was notified of this issue from an end user) our lookups started showing the following error message: "error loading control" as soon as you click in them to populate it. It is not consistent and not specific entities. It is also not isolated to one environment (dev, stage or prod). It is also happening in out of the box entities / forms / fields and custom entities / forms / fields. I have never come across this error before in CRM. I was able to download the error log:

I think is caused by recent updates, possibly after April release.

It might be worth raising a ticket with MS, I have not raised a ticket yet as this issue is on one of client's CRM. I am going to ask them to raise a ticket.
It is. I just got off the phone with Microsoft Support. It is a known issue and should be patched in the June 1st patch. They advised if still having the issue after June 3rd to open up another ticket with them. The current workaround is to refresh the screen.

Thanks for sharing the response from MS. The workaround does not fix the issue with the route queue item dialog as it can't be refreshed. But it should work for lookups on main form I think. Thanks.
