Attachment control component on form cannot add new attachment

Hi All
I've added an attachment control to an order form and enabled add new attachment option as well. However i cannot add NEW attachments as the button is greyed out as below:

    Probably there is an issue with this component.
     
    You can try use another one from here or create one.
