Dynamics 365 Add-in not working as intended

Can anyone help me with this?  When I click the Dynamics 365 Add-in I'm presented with this screen and struggling to follow this article to Set Regarding (Link and track an email or appointment to a specific row in your app (Dynamics 365 apps) | Microsoft Learn). 
 
Any ideas what I'm missing?

    Hi,
    You can try to check if case entity is not setup as read only in UCI.
    --Settings >> Customization >> Customize the system >> Entities >> Case.
    If yes remove this option save and publish your changes.
