Error with case web-form in Portal 365

Hi. I created a Portal (type - Customer self-service Portal) in Power Apps and did a integration with SharePoint (to upload docs from cases right to SP) using instruction https://docs.microsoft.com/en-us/powerapps/maker/portals/manage-sharepoint-documents#step-4-configure-the-appropriate-form-to-display-documents - step by step, so:

Enable document management in model-driven apps - Done;
Enable SharePoint integration from Power Apps portals admin center - Done;
Enable Document Management for entities - Done (includes entities "Case" and "Contact" by the way), and checked that folders in SharePoint site were created;

But next I have a problem... I added a subgrid with document locations on form. Next I need to change mode of "Web - create Case" form to allow upload docs (change mode from "Insert" to "Edit"). I did it, checked "Enable entity permission" and created entity permission for document locations (child to permission "Cases, where's client is contact"), of course I added web-roles (authenticated users) - log in on Portal and there's error...

"The record you're looking for could'nt be found". What's the problem?

When I change form mode back to "Insert", it's okay and works. But I can't upload docs to SP. What to do?
