How can I remove title fields header appear on a form with pattern (Simple List and Details - List Grid) note that title data source property is not determined on the design ?

Look for HeaderInfo group in design of form. there will be "StringEdit:HeaderTitle", and it has either Table method or its value is assigned on Active Method of Datasource. Change accordingly
I try to remove the part marked below in (HcmPersonIdentificationNumber) form ; no header title string field is used . also the tittle fields properties are not determined on the the table itself .
