In Parameter table, can we add custom fields with mandatory requirements?

Yes you can.
Yes you can add new fields by creating extension. If you have too many fields , you can create another table as well.
Because it seems the mandatory fields causing the system not creating the parameter record during the DB Sync.

Of course you can.
Application Suite has such a parameter table, such as CatParameters.
Yes, you can, but, you have to set default value in initValue() method, otherwise you will get errors in new company trying to open parameters form.
