Upgrading from GP 2015 to 2018

I have GP 2015 R2 Version 14.00.1230 and I am trying upgrade to 2018 R2.

Can anyone help for the below my concerns: 

1) Which all patches I need to install in 2015

2) From which version of 2015 I need

3) Which version of 2018 R2 I need to install

4) Also, when I am trying to download patches from Microsoft Customer Resource Portal, the portal is down.

Please help me

    You need to upgrade to 16.00.0716 or later first.
