How to implement risk matrix for a table

Hi, 

Just wondering if anyone knows a good way to implement a risk matrix to a table/form on model driven app, where the user would pick a likelihood (likely, more than likely etc) and then severity for a risk, and it would calculate the risk level on an 'initial risk level' field. Was wondering if anyone knew any efficient ways to implement this?
