Required field not enforced? Why?

I've been developing in Dynamics for 6 years.  I say that so I dont get a lot of basic answers.
 
I have a form with a required field on it.  Which is user lookup field.  It is business required.  Has little red asterik next to it and everything.  The form is published and has been in production for a while.
 
However, after noticing some records without this field populated I started looking into it.  I tested by going in and sure enough I could save a record through the UI without putting this in?
 
I have googled this and havent really found anything besides people talking about using excel or flow or other ways to do this, but not through the UI.
 
So how is this possible?
 
The only small little thing I did find was an article about some google app that is similar and the requiredness wasn't being enforced if the field was a locked field.  Which this one is on my form (because the user is supposed to create the record through a subgrid in the user record which has a relationship...) but for some reason some users go directly to the table with all records in stead of doing it through the subgrid in their record... anyway... 
 
But I can't find any documentation from Microsoft saying this is the same thing as google?  So thought I would ask the community if that is indead the cause or if there is something else?  I'm going to run soemm tests too, but thought I would ask first in case someone just knew.
