Workflow not being assigned to user

I completed my first approval workflow but after submission. It is not being assigned to any user but I have specified a user in assignment. 

In history of my workflow the work items table is empty when I tried to reassign it says "WorkflowWorkItemActionDialog.run has been incorrectly called."

I do not know what I am missing. I found noting wrong in services too. Let me guide to fix this issue
