We created a new button for the ribbon on the lead and opportunity. For some reason, when you load the page initially the button will not show. Once you hard refresh it displays properly.
Has this ever happened to anybody?

You can debug the command bar by passing the ribbondebug=true as a URL parameter and check whether your actions and rules are performing correctly.
