Embedded D365 Marketing Form Does Not Load - Spinning

We are trying to embed a D365 Marketing form on our external website.  We have followed the instructions here: https://docs.microsoft.com/en-us/dynamics365/marketing/embed-forms

1. We authenticated the domain

2. We created the form

3. We created the Form Hosting record

4. We copied and pasted the form embed code onto our web page

5. The form is on the page but it never loads, it just spins and spins and spins

Any ideas?  Here is the loading icon that keeps spinning and spinning.

    It would be appreciated if you could provide more details, such as screenshots, about your steps to embed the form.

    Here is my test result based on your description.

    1. We authenticated the domain

    1. It can be loaded normally.

    Everything you did - except #5, it did not load normally.  It just loads and loads and loads.

    Hi,

    Have you tried to embed the form into another website

    Or embed another form which only contains required fields into the website?

    So that you will know which part of the problem lies.

    we have the same problem with an embedded subscription center form.
    Did you find the cause for this problem or were you able to solve this?

    On my web cms, i just pasted the form ID div at the beginning of the script into a HTML content block on my page, then added the rest of the form script into my page properties header.
    That worked for me with Outbound forms.
    Maybe also check your website CSP content security policy to see that dynamics.com is whitelisted.
