Error When Verifying Domain for Sending Survey

When trying to verify domain for a customer email address from which to send Surveys from, I'm greeted with this screen:

None of those fields ever populate. I've tried logging out and back in, and can't get anything to populate.

Any assistance is appreciated.

Can you share your tenant id and domain name you are using to investigate via email at customervoice@microsoft.com ?
