Hello Everyone,
I created one customer Hourney with following data in it:
- Sgemenent
- Email contains marketing form (externaly created but connected with dynamics using form capture feature)
I sent an email and contcts submitted form
Lead is getting created successfully.
But the problem is when i click on Customer Journey => related => related form submission it keeps showing blank but lead is created using this customer journey
one more thing when i open form submission it shows lead proceed but also show leads in error (PFA)
Can some one guide me how can i resolve this

To track the source journey for the lead, we should build connection between marketing email(customer joourney) and 3rd party hosted page.
The connection would be the encoded link/URL(to 3rd party hosted page) in marketing email, parameters in encoded URL could help script track both source journey and source marketing email.
In your situation, due to there is no connection between marketing email and your Contact Us page, so the source journey couldn't be tracked for the lead.
(Your contact opened Contact Us page via link in the first landing page directly instead of encoded URL in marketing email.)

1. I built a new customer journey as below and got a related form submission record successfully.
I attached the link to capture form page directly in marketing email, because contact opened capture form page via encoded link in marketing email,
thus both customer journey and marketing email are get tracked for the new lead, and form submission record will also track the source customer journey.
Back to your situation, if you would like to also track customer journey for your Contact Us page,
I suggest you could send a new marketing email to contacts, which contains a link to form capture page, let your contacts open and fill Contact Us capture form via link of email.
2. For why KPI still showing Submission errors even lead is generated properly in CRM,
It could be that due to there is no success or error message for us if we submit capture form, so the script for insights service might consider the submission both a success and a failure.

I built a customer journey below and reproduced your issue.
segment -> email -> landing page page/form(The page contains a link to capture form) -> capture form(contact us page)
both form 1 and form 2 are hosted external website.
The fact is that a marketing form submission won't be associated with a customer journey record when it is created,
thus we couldn't see form submissions in related tab of Customer journey form.
It seems that script on web page doesn't know which customer journey it is be involved in.
I will do a more test for you tomorrow.
From another result, the first marketing form knows the submission is from which Email subject
So in my opinion, we might need to associate marketing form with a marketing email directly to make form script could also know the original customer journey, because the marketing form URL has been encoded in the delivered marketing email.
In your situation, your second form was not directly associated with marketing email, thus less data were tracked.
I will test whether directly association could help script to know the orignal customer journey tomorrow.

i also found one behavior,
If landing page is portal hosted (D365 created) then it is tracking form submission in D365 customer journey.
But if my landing page is external hosted then it is not tracking form submission in D365 customer journey.
- Note : End page (contact us) page is configured in D365
