Complex custom lookup Excel add-in

Good morning.

I am transforming a data Entity that is called from Excel. I need customize a lookup field, for that it return two fields and I no know the way for make it. 

Need some like this:

In other fields I have make lookup with single column and it works with the following code:

    Does Excel support lookups with several fields? I don't think I've ever seen any.
