1-Upload (Route Version.csv ) File using Route Entity (Upload Successfully)

2-Then (Upload Route operations - Copy.csv) File  using Route operation Entity ( 3 Row Upload only )

3-Then (RouteRelation - Copy.csv) File using Route Relation Entity (Upload Successfully)

You can set a breakpoint in system Info::add() class method and debug. That will help you to pinpoint why it failed. Normally it should be an update instead of failure. Check what could be causing this situation via debugging.

Are you using a standard or custom(ized) entity? Have you checked the source file for duplicate entries?
