How do I remove the duplicated/repeated fields?
Hi Mbabazi,
Can't tell the form you are on, however, inside you make.powerapps.com environment, navigate to the table, then edit the form:
Edit the form that was customized, and select each field duplicated and delete:
Repeat for each of the duplicated fields in your screenshot.

The current view is the Opportunity Summary.

= > Open that LEad/Opportunity feom Customization.= > Go to that entity. And expand the form then select the form from which you wants to delete the duplicate fields.
= > Select those fields one by one and click on remove.= > After removing duplicate field save and publish the form.
