Print a blank survey

I created a Customer Voice survey in Dynamics 365.  I need to export to MS Word or print a hard copy for review.  I can't find this feature anywhere in Help or documentation.  Please help.

    Hi,

    This features in Customer voice allows survey respondents to print their own responses.

     

     
    Therefore, it is possible to send the survey to yourself and then print it out.
