Error when Adding Dynamics 365 app for Outlook 404 Error and Show Dynamics 365 add-in

I have configured 100+ users for the Dynamics 365 app for Outlook this is the newest app that requires only that the user login to the app in Outlook after being configured in Settings > Email Configuration and Settings > Dynamics 365 App for Outlook.  These configuration and deployments were successful and it shows installed.  However, the user's Outlook displays an odd block in the Ribbon.  Show Dynamics 365 add-in versus the Dynamics 365 that shows the other users Outlook.  Nothing has been installed, nothing in programs and features, no add-ins in Outlook (disabled or enabled).  It is grayed out.  After reconfiguring the user he will get prompted to sign in to our ADFS server, however, he gets a 404 error.  Any thoughts?

    Hi,

    It seems like the old addin is still there in user machine.

    Please check if the users log into outlook owa then they are seeing the right behavior or not.

    Under the addin section of outlook desktop app please check if the addin is enabled, if enabled please disable the same.

    I had that thought too and checked it already.  There is not add-in listed.  Our previous was the "plug-in"  which had a different icon entirely.  There is no trace of that or of what this might be in Control Panel > Features.   I just had a thought to check his C:/Program Files to see if remnants are there.

    There is also no sign on the app or tracking capabilities on outlook.office365.com.
