Quick Filter controls on forms

I have a list page that calls the details page via a menuItem.  When the 2nd page comes up it gives the message A control with the name 'QuickFilterControl' already exists on the form.  I have checked the whole form and there is not another 'QuickFilterControl' name on it.  What could be causing my problem?

    Recently I've faced the same error in a new form I've created,

    issue was super() call was missing in the form init() method.

    hope this helps some one.

    I still have the problem.  Since my form uses the detail transactions pattern, it is making me have a quickfiltercontrol.  I deleted and re-added it and still get message, except now it says QuickFiltercontrol1 instead of QuickFilterControl
