Case form field auto populate based on look-up field

I have a simple table of data(example below) which I want to use when I create new Cases in CRM Service. 
 
 
I have 3 custom fields I've created on the Case form (Instrument Name, Vendor, GitLab Link).  Instrument name is a Look-up field which is working correctly when I search for the appropriate instrument.  What I want to do is have Vendor and GitLab Link fields automatically populate with their respective data from the table based on the Instrument name.  I've been searching for 2 days with lots of information found but no clear path, a simple path, to a solution.  The intent is to not make this so complicated that the team has an adverse reaction to working in Dynamics.  We're trying to get this integrated into our small business processes and we are a small team wearing many hats.
I don't know if I have my table setup absolutely correct but I wanted to start somewhere.  Vendor column has duplicates due to the fact that vendors create many different instruments.  Anyone out here with some clear directions on how to make this happen I'd be glad to listen.  Thanks.

    I followed your steps but I'm not seeing any changes happen.  Here are my screenshots.  Maybe you can point something out that I'm missing.

    May I suggest a JavaScript function to fetch the data from Vendor and Githab based on the Name selection.

    Hi Partner,
    One question, is the table in your first screenshot instrument related data?
    "Cytomat 2" is instrument name? first column in this table?
    If so, you can create one workflow to populate these two fields with value from instrument table automatically.
    1.Go to Settings > process to create one workflow on case table:
    2.Add condition and action:
    3.Save and active it.
