Hi,
When you add Entity in Sitemap directly and not in App Designer. System will Add the entity to App Designer automatically for your.
I tested in My instance. I have added mu custom entity directly in Sales Hub Sitemap editor and when I saved changes I saw entity was added in APp Designer as well.

In fact, when I add one entity to the sitemap, it will appear in App design page automatically.
For the test, I remove it and publish all.
In current, nothing seems to have been affected., the entity appear on the page and you can do any operations on it.
