Hi Rohini,
Add Contact Subgrid in Case Form (Must have N: N relationship between Case and Contact).
Go to Portal > Open your Entity Form > Create one Entity Form Metadata > Select Subgrid in Type > Select your Contac Subgrid > Select Associate in Grid Configuration.
