What process actually created the channel DB tables and indexes? Is it the 'regular' DB sync?
Reason I ask is because we are getting a DB error when activating CPOS, and then event viewer lists a mismatch in the SQL table and expected table by the application (looks like a mismatched index)

It's not the DB sync process. It would the Binary update that will do it for you, The database sync process on takes of the D365 and binary updates take care of both!
you can re-apply the binary update and it should take care of it - assuming that there is no manual change done to the retail schema.
