Management Reporter Attribute fields

I have just installed Management Reporter CU12.  My Attributes in the column layout seem to be missing the Originating Master ID and Name?  Does anyone know if this was removed?  How can I get it back!

    We are on MR2012CU12 as well. But we are on GP 2010. I see a comprehensive list of Book Code/Attribute category fields to choose from including the pair you asked for.
