Tickets created from the portal are triplicated

Hello,

When ticket is created from the portal it's being tripled. Three tickets are created with different GUIDs.

I can't replicate the same issue, because I don't know when it happens exactly, it's something random.

Have you had such an experience? How did you deal with it? What would you advice?
