How is the monthly profit/loss getting transferred to equity?

The Profit/loss gets transferred to equity through running the closing-opening process in the opening form in GL.

Does the profit/loss get transferred at each month end (when we close the month) or at the time of year end closing?

The profit/loss usually does not get transferred at the end of each month but at the end of each fiscal year that you specified.
Only when you run the opening transaction periodic GL process, AX transfers the Profit/loss.
If you want to have a monthly Transfer you would have to Setup monthly fiscal periods. Yet, I have not seen such a Setup and cannot identify a reason for doing that.
