How to set the new phone call form used for click to call

We are planning to implement click to call in the unified interface.  We have a customized phone call main from that we use in our environment and we would like this from displayed when a user clicks on the phone icon for a phone number field on the contact form, rather than having the quick create form displayed.  In our sandbox environments, clicking on the phone icon does open the main form and not the quick create form.  I can't figure out what is different in our non-production environment that allows it to open the main form instead of the quick create.

Does anyone know how to set which form is opened when you click the click-to-call button for a phone number field?

    Hi Bryan,

    Have you checked that the Phone Call entity has:

    1. Allow Quick Create option selected?

    2.  There is an actual Quick Create Phone Call form?

    3. From Timeline, Create Activities Using Quick Create form is selected?

    Allan-

    Thank you for the reply on this.  The Allow Quick Create on the Entity was the issue.  Unchecking this has caused the system to now open the Main for as we intended.  I had thought this was a form level issue and hadn't looked at the entity itself.
