Collecting Respondent Contact Information

In MS Customer Voice, is there a way to ask survey respondents for their name, title, company, email address?  I use Survey Monkey now and it is the Contact Information question and I don't see that capability in Customer Voice. Can someone tell me if this question type is available and how to access it?
